To be fair, this is a bit over the top. The nobles could afford to have baths back in medieval times. They also had servants who dumped chamber pots off the property etc.
The peasants would be the ones without baths, though I am sure they would at least use a bucket of water and a rag to clean occasionally or take a dip in a river. The city streets would be the worst since people would just dump everything out the window.

yeah they could afford baths not often tho ,and the streets were absolutely filth ,just because nobles are nobles it doesnt mean theyd fit into our standard of what noble would be ,the water that people would use was filthy because people would literally use the rivers for everything ,cooking ,bathing ,chucking out their waste ,literally everything ,therfore even if nobles are able to use it it doesnt mean they arent still dirty ,they were cleaner than commoner/peasants but they werent that clean,have u not done history in school?

Actually it depends on which period you consider. Medieval times were much better in term of hygiene compared to 1600s for example please note that king Luis XIV bathe completely just two times in his whole life. There are many narrators that write about his stench and how he tried to cover it trough essential oils. Also it is said that his bed was covered in lice (as his head and hair) and that he had to change clothes at least 3 times a day to smell less apparently.
So yeah, this novel is quite right about some things (unfortunately)
